PRIMARY HEALTH CARE TRUST
We are an 8 doctor practice with a list size of approximately 16,000 patients and with the other GPs in the local area form the Hampshire Primary Care Trust.

COMPLAINTS PROCEDURE
We endeavour to provide the best possible service for our patients. If you are unhappy about the treatment you receive you have the right to complain. There are details of our in-house complaints procedure in our reception area and a leaflet is available on request. If you wish to discuss any concerns you may have pleas do not hesitate to contact the Practice Manager who will be happy to talk with you.

FREEDOM OF INFORMATION ACT 2000
Your rights to information:
In addition to accessing the information in the Publication Scheme you are entitled to request information about Dr Jordan and Partners under the NHS Openness Code 1995.
The Freedom of Information Act 2000 recognises that members of the public have a right to know how public services are organised and run, how much they cost and how the decisions are made.
From Jan 1st 2005 the practice is required to respond to requests about information it holds, whether recorded in electronic and paper format and to create a right of access to that information. These rights are subject to some exemptions, which may be taken into consideration before information is released.
Under the Data Protection Act 1998, you are also entitled to access your clinical records or any other personal information held about you. You can contact the Administrative team, at either site, to do this.

HEALTH RECORDS
We ask you for information so that you can receive proper care and treatment. We keep this information, together with details of your care, because it may be needed if we see you again. The main reasons for which your information may be needed include: giving healthcare and treatment, looking after the health of the general public, managing and planning the NHS, review of care to ensure the highest standard, training and research. Everyone working for the NHS has a legal obligation to keep information about you confidential, and legislation controls the sharing of information.

DISABLED FACILITES
Both surgeries have wheelchair access and toilet facilities for our patients with disabilities.
